As a member, you will also have access to Contact911™, a communication system that allows you to send a message to everyone on your contact list at one time. Whether you want to announce the birth of a child, provide updates on a sick family member’s medical condition, notify loved ones that you are alright following bad weather, or any other reason you may have to communicate with a group of people, you can just make one call to Contact911™ and have your message delivered to everyone on your list. With Contact911™, one call reaches all.

Add the names, phone numbers, and email addresses of people you'll need to communicate with during times of crisis. You can update this information anytime or have CONTACT 911™ keep your contact information up-to-date with our automated maintenance feature.
2) Phone, Email or IM Your Message to Contact911™
You never know where you'll be when disaster strikes, or what communication options you'll have available. With CONTACT 911™ you can rest assured that, as long as you can get to a land phone, cell phone, pager, email, Internet or wireless web, you'll be able to get your message out.
3) Contact911™ Reaches Everyone, Everywhere
In the midst of a crisis, you need to focus on your game plan, not practice your speed dialing skills. CONTACT 911™ relentlessly dials every number for every contact in your list (and emails every email address) until we get through.
4) Stay Connected to Your Family
Family members or employees can respond to your message using the CONTACT 911™ system, ensuring the steady and efficient flow of communication.
Your membership materials will include more detailed instructions on how to setup your Contact List and send messages via phone or e-mail.
These Plans allow you to keep up to six people (depending upon plan selected) in your Emergency Contact Manager and comes with up to 20 minutes (depending upon plan selected) of messaging time per month. At the end of each month, unused minutes automatically rollover to the next month and additional minutes can be purchased from CONTACT 911™.

Global MED-NET stores your vital medical information in its confidential database. Global MED-NET relays this lifesaving information directly to emergency room personnel via fax, which helps to expedite admissions and aids in a more accurate diagnosis. Even if you're unconscious when taken to an emergency room, your Global MED-NET ID helps alert the emergency medical team to call the 24 hour hotline to access your medical history. That's why Global MED-NET is essential for more accurate treatment!
This is particularly important to:
· Keep you and your family safe while traveling.
· Reduce the chance of adverse reaction to drugs.
· Inform emergency personnel who to contact.
Plus, if you're traveling abroad, your vital information can be transmitted to emergency rooms, worldwide, 24 hours a day, 365 days a year.
How To Use Your Program
Simply complete the Global MED-NET brochure in your membership kit and drop it in the mail. You'll receive an identification card, decals, travel stickers, and a copy of your emergency medical information for your verification.
Lifestyle Protector Accident Disability Benefits:

The following limited benefit insurance is provided to all active members of the Contact911™ Plus Lifestyle Protector and is an added benefit of your membership.
If, as a result of an Injury, an insured person is rendered Permanently Totally Disabled within 90 days of a covered accident that caused the Injury, and if the Permanent Total Disability due to that Injury continues for the Waiting Period specified, the insured will qualify for the benefits listed starting with the 1st consecutive month of Permanent Total Disability following the Waiting Period.

All members of the Contact911™ Plus Lifestyle Protector Program are enrolled into the U.S. Benefits Association to be eligible to receive these benefits. These insurance benefits are underwritten by National Union Fire Life Insurance Company of Pittsburgh, Pa (NAIC# 012-19445) and subject to the exclusions, limitations, terms and conditions of coverage as set forth in the insurance certificate provided in your membership materials and Policy issued to US Benefits Association Form No. C11695DBG. All payable amounts are reduced by 50% if member is age 70, or older, on the date of the accident for which benefits are payable.
Hospital Cash Plus Accident Only Coverage:

The following limited benefit insurance is provided to all active members of the Contact911™ Plus Hospital Cash Plus and is an added benefit of your membership.
Lump Sum Hospital Confinement Benefit: If an Insured suffers an Injury that, within 90 days of the date of the accident that caused the Injury, requires him or her to be confined in a Hospital as an Inpatient, the Company will pay an initial benefit after 60 consecutive Days of Confinement due to that Injury and an additional benefit after 90 consecutive Days of Confinement due to that Injury. No benefit is provided for the initial benefit if the Insured is confined for less than 60 consecutive Medically Necessary Days of Confinement. No benefit is provided for the additional benefit if the Insured is confined for less than 90 consecutive Medically Necessary Days of Confinement. A maximum of two single payment benefits is provided for any one accident per Insured regardless of the number of Injuries for which the confinement is required or the number of times the Insured must be confined due to Injuries resulting from the same accident.

Daily Hospital Benefit: If an Insured suffers an Injury that, within 90 days of the date of the accident that caused the Injury, requires him or her to be confined in a Hospital as an Inpatient, the Company will pay the Daily Hospital benefit for each day of Hospital Confinement after 1 Day of Confinement due to that Injury. No benefit is provided for any Day(s) of Confinement that are not Medically Necessary. The daily benefit is payable for a maximum of 365 days of confinement.

Emergency Room Benefit: If an Insured suffers an Injury that, within 72 hours of the date of the accident that caused the Injury, requires him or her to receive Emergency Treatment in a Hospital emergency room or a Satellite Emergency Center, the Company will pay the Emergency Accident Benefit Maximum Amount. Only one Emergency Accident Benefit is payable for any one accident per Insured.

The following limited benefit insurance is provided to all active members of the Contact911™ Plus AD Protector and is an added benefit of your membership.
If an Insured Person dies due to a covered accident within 90 days of the date of the accident the insured will qualify for the benefits listed below:
Base Product Benefits: You will be covered during the following activities:
A. Common Carrier: While riding as a fare paying passenger on a Common Carrier.
B. Motor Vehicle/Pedestrian:
1.) While driving or riding as a passenger in a Motor Vehicle. Injury sustained as a result of participating in this Covered Activity must be due to a collision, wrecking or explosion of such Motor Vehicle. Such collision wrecking or explosion must cause visible physical damage to the Motor Vehicle and leave it in a different condition than before the accident.
2.) While standing or walking, as a pedestrian on an open public street or highway. Injury sustained as a result of participating in this Covered Activity must be due to being struck or run down by a Motor Vehicle while a pedestrian.
3.) All Other Accidents: All Activities except while riding as a fare paying passenger on Common Carrier; while driving or riding as a passenger in a Motor Vehicle; or while standing or walking, as a pedestrian on an open public street or highway.
